The Service and Support Administration plans, develops and oversees services for individuals living with disabilities and their families, provide links to supports and coordinate services, assess needs, facilitate development of individual service plans, arrange for services and oversee the provision of those services.

Helps clients achieve and maintain successful living in the community. Services include family support/education, education and employment referral, linkage to housing and other community resources, assistance with managing finances, training in daily living skills, assistance in obtaining and maintaining benefits, and connection to recovery groups.

Assists public assistance recipients and low income individuals and families in obtaining social services.

Provides specialized services for individuals with a dual diagnosis of mental illness, substance abuse and/or developmental disabilities. Acts as a liaison between the Franklin County Board of Developmental Disabilities and the Franklin County ADAMH system to coordinate services ensuring continuity of care for the client.

Provides case management to HIV positive persons. Offers support and referral to emergency assistance services including transportation, housing placement, nutrition, medical care, diagnostic monitoring, home health care and dental care.

Provides case management for any minor victim of domestic minor sex trafficking. Also conducts outreach and education in attempts to prevent sex trafficking.

The Passport Program provides in-home alternatives to nursing home care for low-income seniors. The Programs are administered through local area agencies on aging.

Provides services to promote family stability; family service plan development, case management and direct services to families

Investigates suspected cases of abuse, neglect, and/or exploitation of Preble County residents ages 60 and up. Also provides case management and community collaboration to alleviate the alleged abuse, neglect, and/or exploitation.

Provides individual mental health assessments and outpatient treatment.

Provides case management support to help seniors remain independent in the community. Services are based on the needs of the individual, but could include: a needs assessment, assistance with applying to government programs (Social Security, HEAP, Medicaid, Medicare), assistance in managing finances, locating and applying for housing, assistance with medical appointment/prescription access, arranging transportation, end of life planning, making linkage and referral to other needed services, supportive counseling and problem solving.

We provide support services for eligible individuals age 3 through adult with eligibility determinations, areas of transition, coordination of benefits, resource links, access to funding, community opportunities, provider search, education, advocacy and employment.

Program provides assistance for older individuals who seek to maintain independence and live in their own homes or apartment.

Senior services include help with citizenship or naturalization, support for medical and legal appointments, social outings with peers, and more.

Provides information and referral service, placement and coordination, crisis intervention, advocacy, major unusual incident reporting and investigating, and service monitoring for agency clients. In addition, assists families with special expenses that may be involved with having a developmentally disabled family member at home.

Provides anonymous and confidential HIV testing. Provides comprehensive case management to people living with HIV/AIDS. Services include emergency financial assistance, referrals for medical, dental, legal, and other community resources, counseling, support groups, activities and rental and utility assistance. Outreach through Ohio AIDS/STI Hotline.

The CARE Clinic offers medical evaluations of children 18 years old and younger where child abuse is suspected or alleged.

Assists consumers in developing, increasing and maintaining stabilization related to their unique combination of mental health and addiction disorders by creating personalized plans to match each person’s needs that encourage motivation and recovery.

Provides behavioral and mental health services including: assessment services, individual and group mental health case management services, individual and group behavioral health counseling and therapy services, pharmacological management, targeted case management, crisis intervention and psychological testing.

Provides psychiatric assessment and treatment, nursing services and support services. Teams include a clinical leader, nurse, psychiatrist and community support services staff to interact with clients. Also offers assistance with entitlements, housing, recreational activities, job search and daily living skills.

Provides behavior and mental health services to youth up to age 21. Services include assessment, case management, individual and group counseling, pharmacaological management, crisis intervention and psychological testing.

Provides system of care for Franklin County older adults to assist them in remaining independent. Provides information, advocacy and case management. Will link older adults to resources such as homemaker services, personal care, caregiver relief, emergency response systems, home delivered meals, medical transportation, adult day services and minor home repair.
