Fcf Service Coordination/Wraparound Services | Washington County Family And Children First Council

The purpose of Service Coordination and Wraparound Services through the county FCFC is to provide a neutral venue for families requiring services where their needs may not have been adequately addressed in traditional agency systems. These processes serve as a safety net for children needing a more intensive collaboration of multi-system providers. Each system has areas of responsibility, and the Service Coordination Mechanism is not intended to override current agency systems, but to supplement and enhance supports that currently exist, or identify additional supports that are needed but are not currently utilized. The goal is to keep children in their home and community with supports.

Provided through a partnership with Compeer International. Opportunties in the program may include: - One-to-One Friendship Program: Offers face-to-face contact with adult volunteer. - Compeer Calling: Provides telephone support (may be offered in addition to face-to-face support). - Compeer Community: Offers group events and celebrations among Compeers to strengthen social skills. - Peer-to-Peer: Matches clients with one another in supportive, one-on-one friendships. - Skillbuilders Program: Promotes social ties through structured life skills training.

Chat Pals' mission is to promote friendship through developing caring connections with others. Provides a telephone-based or video-based (Facetime or Zoom) program designed to be a support system, providing emotional support and companionship to people in the community. The purpose of the program is to bring people together based on like-minded interests and experiences so that a friendship can be formed remotely. By developing friendships, it will combat the negative effects of social isolation and loneliness.

Help Me Grow Home Visiting | Washington County Family And Children First Council

Help Me Grow Home Visiting is Ohio's program for expectant parents, newborns, infants and toddlers. The program provides health and developmental services so that children start school healthy and ready to learn. Help Me Grow works with parents to support young children's development. Home Visitors provide on-going home visits in the home; focus on improving prenatal health habits and infant care giving skills; provide information on child's health, growth, education and development through the Parents As Teachers program; provide screenings for health, hearing, vision and development; connect families with community resources and services. Affiliated with the evidence-based home visiting model Parents As Teachers. Services include - Developmental Screening - Information on parenting tips - Assistance in developing and reaching family goals - Home Visits to help each child stay "on the road" to healthy growth and development - Service Coordination - professional nurses, teachers, or other qualified staff who share their knowledge about available resources, assist families in connecting with needed services, coordinate services to help families meet their needs and goals. Volunteer based program that regularly checks on the safety and well-being of clients. Determines if there is a follow-up referral to an appropriate agency to improve the client’s quality of life is needed. Assists in providing any immediate help needed, within program guidelines, during actual visitation. Also checks residences at the request of citizens that are away from their homes for extended periods of time.
