Provides inhome companionship to seniors 55+ older who simply need a friend and to combat senior isolation

The purpose of Service Coordination and Wraparound Services through the county FCFC is to provide a neutral venue for families requiring services where their needs may not have been adequately addressed in traditional agency systems. These processes serve as a safety net for children needing a more intensive collaboration of multi-system providers. Each system has areas of responsibility, and the Service Coordination Mechanism is not intended to override current agency systems, but to supplement and enhance supports that currently exist, or identify additional supports that are needed but are not currently utilized. The goal is to keep children in their home and community with supports.

Provides visitors to call on people who are hospitalized or in another institutional setting or who are socially isolated with the objective of brightening their day and helping them to maintain contact with the outside world by reading, talking, listening, writing letters, or performing other similar tasks.

Personal visits are made to the person in the home, hospital, or nursing home. To provide regular communication concerning the person's welfare.

Offers a variety of senior companion programs and services for isolated elderly who have little or no local family support, including one-to-one visiting, social outings, events and holiday parties.

Provides monthly telephone contact for people with cancer to listen and talk.

Provides a peer support program to match adults coping with severe mental illness with a community volunteer for friendship and social skill building. Provided through a partnership with Compeer International. Opportunties in the program may include: - One-to-One Friendship Program: Offers face-to-face contact with adult volunteer. - Compeer Calling: Provides telephone support (may be offered in addition to face-to-face support). - Compeer Community: Offers group events and celebrations among Compeers to strengthen social skills. - Peer-to-Peer: Matches clients with one another in supportive, one-on-one friendships. - Skillbuilders Program: Promotes social ties through structured life skills training.

First Tuesday Tea is an informal forum that allows seniors to actively engage with visual art, music, movement, literary interests, as well as social and current issues, while sipping a hot cup of tea.

Offers friendly visiting to residents of Greene County who are over age 50. Volunteers or staff members monitor the emotional and physical state of the client and encourage social interaction.

Helps reduce social isolation for older adults by arranging for a "buddy" to telephone them on a regular basis to listen, talk, and help them maintain contact with the outside world. Accepting applications for callers and those who would like to receive calls.

Provides a program to protect senior citizens. Also provides a telephone check-in program designed to protect citizens that live alone.

Provides outreach services which include transportation to medical appointments; home visiting; assistance with tax and insurance forms and applications. Also a site for the Golden Buckeye program.

Chat Pals' mission is to promote friendship through developing caring connections with others. Provides a telephone-based or video-based (Facetime or Zoom) program designed to be a support system, providing emotional support and companionship to people in the community. The purpose of the program is to bring people together based on like-minded interests and experiences so that a friendship can be formed remotely. By developing friendships, it will combat the negative effects of social isolation and loneliness.

Provides home visitation, senior socialization, advocacy for social services, and senior ride program.

Outreach services provide older adults with the assistance needed to access the human services system and provide linkage between client and needed services. This service includes management of a home delivered meal program, telassurance, public benefits assistance, housing placement assistance, friendly visiting, and assessments

Pairs volunteers with children to provide socialization, life building skills and friendship. Visits take place either weekly or twice a month, depending on the family's interest.

Provides volunteer support for breast cancer patients.

Help Me Grow Home Visiting is Ohio's program for expectant parents, newborns, infants and toddlers. The program provides health and developmental services so that children start school healthy and ready to learn. Help Me Grow works with parents to support young children's development. Home Visitors provide on-going home visits in the home; focus on improving prenatal health habits and infant care giving skills; provide information on child's health, growth, education and development through the Parents As Teachers program; provide screenings for health, hearing, vision and development; connect families with community resources and services. Affiliated with the evidence-based home visiting model Parents As Teachers. Services include - Developmental Screening - Information on parenting tips - Assistance in developing and reaching family goals - Home Visits to help each child stay "on the road" to healthy growth and development - Service Coordination - professional nurses, teachers, or other qualified staff who share their knowledge about available resources, assist families in connecting with needed services, coordinate services to help families meet their needs and goals. Help Me Grow is - Family-friendly and family centered - Supportive and respectful of each family's needs and preferences - Individualized to meet the needs of each child and family.

Provides one-on-one companionship or daytime respite care to older adults and/or individuals with disabilities to enable them to remain in their home and community. Purpose is encouragement to accomplish household tasks and socialization for participants. NOTE: no overnight services available.

Friendly Visiting program matches screened, trained volunteers with lonely and isolated seniors. Seniors benefit emotionally and physically from the warm, caring connection, closer ties to the community, and more access to services to help them stay safe and independent.

Volunteer based program that regularly checks on the safety and well-being of clients. Determines if there is a follow-up referral to an appropriate agency to improve the client’s quality of life is needed. Assists in providing any immediate help needed, within program guidelines, during actual visitation. Also checks residences at the request of citizens that are away from their homes for extended periods of time.
