Provides a plastic container (large medication bottle) containing forms on which client can record pertinent medical and personal information. The container is to be kept in the home refrigerator, and stickers are provided to alert EMS personnel to its existence. In an emergency this information is readily available to EMS personnel and/or neighbors.
