Provides aides to complete homemaker tasks such as light housekeeping, grocery shopping, laundry, seasonal deep cleaning and errands (prescription pick-up, etc.). While in the homes, staff assesses each situation to ensure safety and cleanliness. Each individual has a personalized plan of care to outline their needs, which is updated with the participant on a regular basis. Persons receiving the assistance are asked to provide supplies necessary to complete the tasks requested.
Provides case management and coordination of needed home care services in the following areas: medical transportation, emergency response system, personal care, homemaker services, home delivered meals, and chore services, including laundry service. Services can be requested individually or combined. Can provide limited home bathroom modifications for safety. Does not include skilled nursing services. When program is full, assists those on waiting list to find needed services in the community. Chore services and limited bathroom modifications cannot be assessed without using other services.
