Assistance is provided with household chores and personal care such as getting dressed and keeping clean; making forod; cleaning and doing laundry; getting out into the community and going to medical appointments.

Provides light housekeeping or heavy cleaning services depending upon the needs of the client. Includes services such as full house cleaning, scrubbing floors, bathroom and shower cleaning, dusting, vacuuming, laundry, grocery shopping, and cleaning cupboards. Will also pack, unpack, and organize home during a move. Will not clean outdoor windows requiring the use of a ladder or perform full carpet cleaning.

Provides non-medical needs and companionship of the elderly and those recuperating from injury or illness.

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides a Medicaid-funded program that offers home health care and services coordination. Services include personal care, homemaking, emergency response system, home-delivered meals, adult day services, medical transportation, counseling services, medical equipment and supplies, and long-term care planning.

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Provides a range of home health care services including but not limited to skilled nursing services for diabetic and cardiac care, as well as assistance for older adults and those with a chronic illness or disability. Also provides additional support, such as rehabilitative therapies and social work services.

Offers home based primary care and nursing services to eligible veterans. Also provides Homemakers and/or Home Health Aide who are trained persons who can come to a Veteran's home and help the Veteran take care of themselves and their daily activities. Homemakers and Home Health Aides are not nurses, but they are supervised by a registered nurse who will help assess the Veteran's daily living needs.

Provides the following services to anyone in need, regardless of his or her ability to pay: * Medication for pain and symptom management, * Professional nursing visits, * Assistance and instruction to family caregivers, * Personal care and homemaker assistance, * Medical and social services, * Spiritual care from the hospice chaplain, * Dietary guidance from a registered dietitian, * Physical, occupational, speech and music therapies, * Trained volunteers, * Bereavement support, * Short term inpatient care, * On-call professional staff, and a supplemental paid caregiver fund.

Coordinates services through local providers to assist seniors with activities of daily living, personal hygiene, or with routine tasks to maintain a clean, safe, and healthy living environment. (housecleaning, cooking, etc. - small jobs to assist the individual to stay in their home)

The Passport Program provides in-home alternatives to nursing home care for low-income seniors. The Programs are administered through local area agencies on aging.
