Physical, Occupational and Speech Therapists Therapists provide a range of rehabilitation services aimed at improving activities for daily living. Home Health Aides Aides help with personal care, meal preparation, light housekeeping and the activities of daily living. Homemaker Individuals assist with light housekeeping, meal preparation, grocery shopping and other errands. Companion Caregivers share time together, participate in activities, supervise and assist with meal preparation. Social Services Medical social workers assist with financial and social needs and help in connecting with community resources.

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.
