A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Arranges for and/or funds routine household and personal care activities for people with developmental disabilities through various programs. Provides skilled home health care services to residents of Montgomery County. Services include: nursing; physical, occupational, and speech therapy; nutrition; enterostomal therapy; intravenous (IV) therapy; social work; and home health aides.
