Western Reserve Navigator Palliative Care | Hospice Of The Western Reserve

Provides palliative pain and symptom management, care, and emotional support for persons with a life-limiting illness (life-threatening or terminally ill) and their families. Provides supportive services such as bereavement counseling, social work, art and music therapy, and spiritual care. Respite care is provided for client's families.

Provide charitable support to the community's chronically and terminally ill children and their families. Also provides assistance through General Fund Grant which provides up to $5,000 and is available for any non-profit organization that provides services for children with chronic or terminal illnesses.

Helps children in the community who are chronically or terminally ill and supports their families during difficult times. Offers up to $2,500 in financial assistance to approved families caring for a sick child. Through its DreamCatchers for Kids program, the foundation creates meaningful experiences that bring joy, comfort, and hope to children facing serious medical conditions. Does not cover Mortgage/rent expenses, utility bills, insurance-covered costs, and non-medical expenses.
