Dietary accommodations include kosher, diabetic-friendly, gluten-free, renal, and cardiac therapeutic meals. Case managers work closely with clients to address nutritional needs.

Program provides in-home alternatives to nursing home care for eligible individuals. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides two meals daily for older adults and disabled/convalescents who are unable to shop for and prepare their own food. The meals include one hot and cold meal, both delivered in the middle of the day. This is a private pay program. Diabetic individuals (not insulin injection dependent) and individuals needing a low sodium diet can be accommodated by arrangement.
Offers three options of home delivered meal including hot meals, frozen meals and shelf stable meals. In addition to a nutrient rich meal, our home delivered meals serve as an opportunity for us to connect with members of our community whom are often underserved. For those who like to be out and about, we offer a dine-in option Monday to Friday 11:30 to 12:30 at our office in Upper Sandusky.

Home delivered meals are available for anyone 60 years or older who is homebound by reason of illness, incapacitating disability or is otherwise isolated. One must be physically unable to obtain food or prepare meals and there can be no adult living at the same residence that is willing to prepare meals. An individual who resides at home with an individual who is determined eligible may also receive a home delivered meal. Meals are delivered throughout most of Hocking, Athens, and Perry Counties.
Provides two home delivered meal options. Title III provides one hot meal delivered each weekday. Private pay provides two frozen meals delivered each day. Title III can accommodate diabetic diets; private pay can accommodate any diet required with prior arrangement.
Provides up to 28 home delivered meals to mothers after the birth of a child.

Prepares and regularly delivers meals to older adults in Warren County.
