Offers home delivered meals, both hot and frozen, for people who are homebound, unable to prepare meals, or have no one to help. The need may be temporary or long-term. Regular, modified, therapeutic, kosher, and emergency blizzard box meals are available. Case managers work closely with clients to address nutritional needs.

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Home Delivered Meals provide daily nutritional needs to participants and it serves as a safety feature as volunteers delivering the meals are able to check on the welfare of an older adult and provide companionship. Hot meals are delivered to a consumer living within ½ mile of the city limits of Bucyrus, Crestline, or Galion. A choice of an entree and the sides that accompany it are offered Monday-Friday. Eligible consumers living outside the 1/2 mile radius of the above mentioned cities receive frozen meals delivered weekly to their home.
Delivers nutritious, freshly prepared, hot lunches (one meal per day) Monday through Friday, including holidays, to Piqua residents unable to provide for themselves due to age, illness or other inability. Meals are prepared to accommodate clients' dietary needs/restrictions based on doctor's orders.

Home delivered meals are available for anyone 60 years or older who is homebound by reason of illness, incapacitating disability or is otherwise isolated. One must be physically unable to obtain food or prepare meals and there can be no adult living at the same residence that is willing to prepare meals. An individual who resides at home with an individual who is determined eligible may also receive a home delivered meal. Meals are delivered throughout most of Hocling, Athens, and Perry Counties.
