Pre-Admission Screening System Providing Options And Resources Today (PASSPORT) | Catholic Social Services Of The Miami Valley

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Project Hope (HOME OWNERSHIP PREVENTS EVICTION) - Mobile Homes | Community Action Council Of Portage County

Provides needed repairs and handicapped accessibility modifications for mobile-home owners. Funds may be used to repair or replace a heating unit, hot water tank, severely deteriorated windows, walls and doors, or correct code violations. May also be used to improve accessibility for those with disabilities. In some cases, may be used to improve sewer, well, and plumbing.

The Habitat Critical Home Repair program helps low-income homeowners by providing affordable loans to help repair critical issues in or around their home. Through the program homeowners' partner with Habitat for Humanity and our volunteers. This work improves quality of life and preserves the housing stock in Southeastern Ohio. Note: This is not a free program. The Home Repair program addresses health and safety concerns in several areas of the home as well as in the community. Projects have a financial cap of $5,000. They include, but are not limited to: - Unsafe Roofing - Sidewalk Issues - Plumbing Issues - Roof/Deck Repairs - Accessibility - Hazards inside and outside the home.

Microfinance Loan Program | Habitat For Humanity International

The Microfinance Program can provide affordable loans up to $2,000 so families can improve and maintain their homes and their quality of life without seeking out payday or high-interest predatory loans. Loans available for: - Replacement of countertops, cabinets, flooring, doors, windows, etc. - Repair (if manufactured within the last 15 years) or replacement of appliances - Repair of porches, decks, roof, etc. - Painting - Upgrades for energy efficiency - Accessibility projects (e.g., ramp). - Vehicle Repair funds may be available if the automotive repair could be linked to staying housed (example: fix vehicle to get to work to pay for housing). Funds may be used to address “quality of life” issues for Southeast Ohio residents in order to maintain and sustain their current housing.

Mansfield Community Development - Emergency Home Repair | City Of Mansfield

Provides and administers the Community Development Block Grants. Works with the intent of assisting persons of low and moderate income, (regardless of their financial means) to live in a decent, safe, and sanitary environment by eliminating urban blight and detrimental living conditions. Provides funding for home rehabilitation. Common examples of housing services we have assisted with in the past: roofing and gutters, porches and stairs, water heaters, furnaces, exterior doors, and electric upgrades. The financial limit for an emergency rehab project is $22,500.

Community Housing Impact And Preservation (CHIP) | Lorain County Community Development Department

Offers home repairs and rehabilitations to income-eligible Lorain County homeowners. Repairs may include roofs, water heaters, furnaces, A/C, electric panel, electrical, ADA modifications, plumbing, and more. The Home Repair Program provides no-cost upgrades and fixes for qualified homeowners. This grant-funded program requires no repayment by the homeowner. The Home Rehabilitation Program offers more extensive repairs and upgrades for homes. Assistance will be in the form of a declining-forgivable loan. There is no monthly payment, and no interest will be charged. No credit check is needed. 100% of the amount borrowed will be forgiven over (5) years provided owner continues to own and occupy the home. NOTE: Funding varies from year to year and might not always be available.

Family Home Based Services | Geauga County Board Of Developmental Disabilities

Facilitates financial and technical assistance to families who need to purchase special equipment for a family member, or purchase and use special dietary supplements or diapers prescribed by a physician or dietitian. Provides financial assistance toward the installation costs of necessary home modifications for those with disabilities, and also to pay for counseling, training, and education for family members caring for an individual with developmental disabilities.
