Offers in home medical care for patients who have a difficult time visiting a doctor's office including geriatric and homebound patients. Provides most of the same services patients would receive in a doctor's office but in the patient's home setting. Services include physical examinations and treatment, medication management, laboratory testing, mobile x-ray, cardiology and pulmonology consultation, transitional care expertise, and home care coordination.

Home Health Aides to stay with person needing minimal personal assistance but some help with bathing, meal preparation or light housekeeping, errands, 24 hour minimum but no minimum on days used. Can request service on occasional or regular basis. We will fit the hours and program to the client's needs. Supervised by RN at no cost with RN on call 24 hours a day. Also can make visits every two weeks or more often if necessary.
