PASSPORT is a statewide elder care program designed to provide a voluntary alternative to nursing home care. There is no charge for the assessment and people who are eligible for PASSPORT home care may receive in-home services.
Provides an integrated program of home care services designed to address the specific home care needs of those under the age of 60 needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination. (Upon turning 60, waiver recipients are automatically transferred onto the Transition II [Carve-out] Waiver, which is only available to these individuals, and cannot be applied for independently)

Provides programs that assist migrant workers in developing the knowledge and skills needed to enable them to make decisions regarding their job conditions, compensation and human rights; also provides assistance in meeting social and economic challenges and supporting the rights of the migrant worker; a mobile medical van provides basic medical services to migrant farm workers; provides immigration counseling, referrals to housing, transportation and food needs to immigrant populations.

Once PASSPORT receives a referral, a licensed social worker or registered nurse will visit to determine needs and provide the individual with information about PASSPORT home care, community-based services, or nursing home placement.
Provides skilled home health care services (nursing and therapy) and other related managed care services.
Provides primary, preventative, diagnostic, and urgent health care services to homebound adults who cannot otherwise access regular medical care. Program involves physicians, nurse practitioners, and clinical social workers.
