Eligible services include: adaptive and assistive equipment, assistance in the home with health activities such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Two types of benefits packages are available: Home Health Services, and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services as Core Services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination.
