Provides a 24/7 crisis hotline for people whose emotional distress may range from difficulty coping with daily pressures to a psychotic episode. Includes immediate assistance for those experiencing suicidal feelings.

They provide in-home nursing assessment, home evaluations, nursing treatments, and client and family education.

Provide a higher level of personal care, shaving, bathing, skin care, catheter care, colostomy care, turning and positioning, assistance with ambulation, transfers to wheelchair, etc. Nurse aides can do vital signs and remind patient to take medication as set up by RN. Available minimum of 2 hours up to 24 hours a day by 8 hour shifts. Supervised every 2 weeks by RN. RN on call 24 hours a day. Plan of treatment submitted to physician. Also offer companion and homemaker service.

Provides personal care services that combine a variety of health care and custodial care services in the home; some services include bathing, dressing, meal preparation, laundry, homemaker duties, errands, all aspects of personal care, and companion and support services; also may include nursing services, such as medication set-up, medication reminders and medical assessments.

Provides a range of home health care services including but not limited to skilled nursing services for diabetic and cardiac care, as well as assistance for older adults and those with a chronic illness or disability. Offers personal care and homemaker assistance, including bathing and grooming, light housekeeping, laundry, assistance with daily living tasks, companionship, meal planning and preparation, errand running, and transportation/escort to places such as medical appointments, shopping, and family functions. Also provides additional support, such as rehabilitative therapies and social work services.

Provides a full array of personalized home care services including Homemaker/Home Health Aide and Companion Services, Medical-Surgical Nursing, Pediatric Nursing, and Mental Health Nursing. Our trained nursing assistants can provide individualized care to patients in the privacy of their homes. Services include: Assistance with bathing (activities of daily living); Meal Preparation; Light Housekeeping Services; "Tuck in Service" assisting patients with bedtime preparation; Shopping Services; Medication Reminder; Companion Services.

Provides a variety of services that enable individuals to remain independent and cared for at home. Included are planning for families with a member who requires an increased/increasing level of care; recommendations and referrals to address problem areas; counseling to deal with life change, loss, anxiety and depression; physical, occupational, or speech therapy; skilled home nursing; personal emergency response systems that alert a remote agency when someone has fallen and requires assistance. Service includes caregiver training.

Home visits are available for tuberculosis and respiratory patients. For complete homebound patients, home visits are available including, flu and pneumonia vaccines, medication assistance, air conditioner and respiratory equipment (with a physician’s order). Provides services if enrolled in their programs, a case manager helps ensure an individuals needs are met with home and community based services. These services are delivered through various case management programs and through a network of providers and may include but not be limited to help with personal care, housekeeping, preparing or receiving home delivered meals, transportation, adult day care and home medical equipment.

Provides primary health care to eligible veteran patients in their homes. HBPC is a mobile service that is designed to meet your medical, nursing, social and rehabilitative needs in your home. The program provides instruction, support, and reassurance, but is unable to provide bathing, other personal care, or housekeeping services. Other services available when requested by the team.

Services include comprehensive reviews of medical history, physical examinations and ongoing treatment, medication management, laboratory services and portable diagnostic testing, referral to physical therapy and rehabilitation services and coordination of home care services.

Delivers primary health care in the home through a hospital-based interdisciplinary team to homebound (often bedridden) eligible veterans, whose caregivers are willing and able to assist in their care. Provides medical and nursing care, rehabilitation services, dietetic consultation, psychosocial assessment, educational services, medications, and equipment.

Offers in home medical care for patients who have a difficult time visiting a doctor's office including geriatric and homebound patients. Provides most of the same services patients would receive in a doctor's office but in the patient's home setting. Services include physical examinations and treatment, medication management, laboratory testing, mobile x-ray, cardiology and pulmonology consultation, transitional care expertise, and home care coordination.
