Home visits are available for tuberculosis and respiratory patients. For complete homebound patients, home visits are available including, flu and pneumonia vaccines, medication assistance, air conditioner and respiratory equipment (with a physician’s order). Provides skilled home health care services to residents of Montgomery County. Services include: nursing; physical, occupational, and speech therapy; nutrition; enterostomal therapy; intravenous (IV) therapy; social work; and home health aides.

Provides a 24/7 crisis hotline for people whose emotional distress may range from difficulty coping with daily pressures to a psychotic episode. Includes immediate assistance for those experiencing suicidal feelings.

Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.

Provides both skilled and supportive services to homebound individuals who are under the care of a physician. A team of health care professionals, including nurses, nurse aides, social workers and dietitians, as well as physical, occupational and speech therapists, will work with the personal physician to help restore, promote, or maintain a person's health. Such assistance helps prevent the need for a trip to the emergency room or admission to the hospital.

PASSPORT is a statewide elder care program designed to provide a voluntary alternative to nursing home care. There is no charge for the assessment and people who are eligible for PASSPORT home care may receive in-home services.

The Passport Program provides in-home alternatives to nursing home care for low-income seniors. The Programs are administered through local area agencies on aging.
