Provides personal care services that combine a variety of health care and custodial care services in the home; some services include bathing, dressing, meal preparation, laundry, homemaker duties, errands, all aspects of personal care, and companion and support services; also may include nursing services, such as medication set-up, medication reminders and medical assessments.

Provides a 24/7 crisis hotline for people whose emotional distress may range from difficulty coping with daily pressures to a psychotic episode. Includes immediate assistance for those experiencing suicidal feelings.

Home and Community Based Services Waiver options provide home health care to individuals who wish to stay in their home, but otherwise need institutional care. There are several types of waivers: Ohio Home Care Waivers meet the home care needs of individuals up to age 60 and whose medical condition would otherwise require them to live in a nursing home or other institution.; PASSPORT Waivers provide in-home services to individuals aged 60 and older.; Individual Options and Level One Waivers provide support services for Individuals aged 60 and older.; Assisted Living Waivers offer more supervision and services than what may be available in a traditional home setting and allows consumers to have more independence and fewer restrictions than a nursing facility.

A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community. Arrangements are made for services such as home health aide, home delivered meals, adult daycare, homemaking, and participant directed services etc. Call (800) 521-6419 to schedule an appointment for assessment.

Our trained nursing assistants can provide individualized care to patients in the privacy of their homes. Coordinates care to help older adults who are in need of long term in-home health care care to remain in their home and avoid frequent hospitalizations or nursing home placement. Works with the patient, their family, and medical service providers to meet the individual needs of the patient and help them to remain as independent as possible. Agency can provide in home nursing care as well as in home health aid services, but will work with outside agencies based on the client's preference. Transportation to adult day care is provided. Dental care, vision care, respite care, rehabilitation services and hospice care can be provided by the agency as needed. If a participant needs any Durable Medical Equipment, such as a wheelchair or a lift chair, that equipment is ordered, delivered, and installed free of charge.
