The Care Team: - lifts spirits, - promotes connectedness, - walks with our neighbors as they navigate life's challenges, - provides help in urgent times of need, - mentors and shares wisdom and expertise, - offers a listening ear and a safe space.
Provides diagnostic assessment, Strategic Family Therapy, targeted case management, school and community intervention. Designed for complicated cases and difficult issues such as: delinquency, school failure or refusal, anger management, Children Services involvement, self-injurious behavior, Serious Emotional Disturbance (SED), major mental illness, mood disorders, acting out, oppositional behavior and ADHD.

Provide Pack N Play portable cribs and Safe Sleep Survival Kits to families and caregivers that are unable to provide a safe sleep environment due to financial hardship. Survival Kit contains a sheet for the Pack N Play, a sleep sack for the infant, a bedtime storybook and educational materials for the parents. Family must come to Board of Health office to pick up Pack N Play and receive instruction on proper assembly and use before taking Pack N Play home.

Family and Senior Services overview of programs supporting families: Counseling: One-in-five Americans struggle with diagnosed mental health issues. Yet, mental illness is still stigmatized in our society. When mental health and negative stigmas are paired with poverty and hardship, the most vulnerable among us may have the most difficulty accessing vital mental health resources. CSS provides high-quality mental healthcare to low-income clients who are school age and older, because we believe income should not be a barrier to accessing mental healthcare. We employ licensed social workers and our counseling program is nationally accredited by the Council on Accreditation (COA) and certified by the Ohio Department of Mental Health and Addiction Services. Maternal Counseling: CSS’ Maternal Counseling program allows our licensed social workers to provide clinical counseling to mothers in their homes. This is essential for helping low-income mothers—many of whom experience pregnancy- or parenting-related stressors such as postpartum depression, relationship problems, isolation, or a lack of support. We have seen through our experience with this population that new mothers often have multiple barriers to care and lack the resources they need to raise their children in safe, nurturing environments. We are one of the only agencies that visits these mothers in their homes to remove barriers to care, like finding childcare and transporting children. We are committed to giving low-income mothers the support they need to keep themselves and their children healthy. Case Management: CSS’ case manager is available to help clients tackle the multitude of barriers to economic self-sufficiency and overall family well-being. Our case manager can help clients acquire affordable housing, learn how to manage their finances, find childcare, find jobs, access benefits and community resources, and more. Our case manager acts as an advocate and support for clients who lack strong support systems. Our Lady of Guadalupe Center: Through Our Lady of Guadalupe Center, CSS works to reduce poverty by addressing its root causes within the immigrant Hispanic community concentrated on the west side of Columbus. This population is growing and experiences disproportionate poverty. In 2017, CSS expanded the Guadalupe Center facility, which allows us to serve more people. In addition to meeting their emergency needs, we now offer holistic case management and wrap-around services which will transform our clients’ capacity to become self-sufficient Pathways to Hope (Pathways): CSS’ Pathways to Hope program addresses homelessness and domestic violence in the Columbus, Ohio area. Pathways steps in to empower survivors of domestic violence with children by offering two years of case management. By supporting survivors and their children as they leave the homeless shelter, we help them rebuild their lives in safe homes, with stable incomes and a network of healthy, supportive relationships. Stay the Course™: CSS will place three case managers at Columbus State Community College starting in August 2019 to replicate Catholic Charities of Fort Worth’s trademarked Stay the Course™ program. Case managers will help low-income students persist in school and ultimately graduate, equipping them with credentials that increase earning power. The Wilson Sheehan Lab for Economic Opportunities (LEO) at the University of Notre Dame will evaluate the program’s results. St. Francis Center: The St. Francis Center serves Portsmouth, Ohio as a one-stop community center providing families hot meals, clothing and household items, food, and emergency assistance. The Center serves as hope and healing for the community. Provides social work services for those utilizing Veterans Affairs medical facilities. Social Workers are available to assist in a variety of settings to include the primary care clinic, during hospitalization, at Emergency Department visits, in mental health and rehabilitation units, and upon enrollment in specialty medical programs. Will assist the veteran, their family members, and Caregivers in resolving housing, finance, mood, relationship, and physical limitation issues in individual, group, or family treatment, whichever suits your needs. Social workers often provide long-term case management services to veterans who are at high risk of being admitted to a hospital, those who have very complex medical problems, and those who need additional help and support.

Provide referrals and resources when available by assessing individual and family needs, and applying appropriate resources when available to those who qualify, along with giving appropriate referrals when needed. Includes referrals for food and clothing, managing finances, pay bills or maintain a suitable residence.
Provides culturally specific and trauma-informed services to individuals experiencing domestic violence, sexual assault, and/or human trafficking, with a focus on the immigrant and refugee communities. Both advocacy and legal services are available. The advocate may be able to help with crisis and long-term safety planning, counseling referrals, financial planning, support and referrals to shelter, case management, filing for a Civil Protection Order (CPO), and other services. Attorneys are available when applicable to meet with clients to discuss any legal concerns. If qualified, attorneys can help with a divorce, CPO representation, Marsy's Law, immigration, custody, and so forth. Provides a holistic approach model of assigning one advocate and one attorney to help the client through all their needs, which is beneficial for all survivors, preventing the survivor from having to relive their trauma over and over again by telling multiple people the same story. Offers support groups for different ethnic communities and education/outreach to community members, service providers, and others specializing in cultural humility education. Service is not for those in immediate emergencies, please call 911 if in immediate danger.

Provides a comprehensive program which, for eligible families, provides case management for pregnant women, infant care, and parenting education. The children are in a group care setting with a very low caregiver to child ratio, where they are stimulated by a developmentally appropriate curriculum. They also are directed to needed immunization services and health care. All children are given a developmental screening and referred to supportive services based on the results.

Provides case managers from both Su Casa Hispanic Center and Refugee Resettlement Services to assist individuals and families in navigating complex systems by advocating for them and connecting them to much needed resources in the community, such as health, housing, employment, education, legal, and mental health services.
Helps individuals and their families explore options and develop plans to care for older adults. Program helps to identify a loved one’s strengths, needs, concerns and preferences; to establish goals and implement a plan of care; to connect individuals to community resources and government benefit programs; to coordinate doctor appointments, transportation, and caregiver services. Services include counseling to help cope with issues associated with aging, such as changes in independence, health problems, dementia, death of a loved one, isolation, and family dynamics. Counseling is also available for coping with mental health issues, such as anxiety and depression.
Provides employment and training services to eligible, low-income individuals based on a comprehensive assessment of employment and training needs, as well as a basic skills assessment. Participants are provided services to support goals outlined in their individual opportunity plan, which may include support to obtain a high school diploma, job placement, work experience, and other supportive services such as rent, child care and transportation. Participants are provided with case management to address barriers to self-sufficiency such as health, addiction/substance abuse issues, housing, education, transportation, etc. Participants are typically offered paid Summer employment.
