Preschool Referral Programs
The purpose of the Adult Foster Home Program is to assist aged, blind & disabled adults who need some assistance or supervision but do not require skilled nursing care. Residents receive help with personal care to remain in the community in a safe and supervised environment. Adult Foster Home providers are individuals or families with the desire and ability to help others. Providers share their home and family life with either one or two residents. Being a provider involves a commitment to provide room, meals, accommodations and personal care to the residents living in the home. Adult Foster Homes must conform to prescribed guidelines on space, furnishings, nutrition, safety and cleanliness. Caregivers may have to reconfigure their homes in order to meet such guidelines designed to protect both the operator and the resident.
Arranges placement of children, who have been court-ordered into the agency's custody because of abuse/neglect/dependency, into licensed foster placement sites until children can be reunited with family or adopted. Identifies, enlists, trains, and certifies adults who are willing and qualified to provide foster care services in their homes for children in agency custody. Provides on-going, supportive services to foster parents; children in foster care; and the children's families of origin who are working on reunification plans.

Child care referrals are provided to local family child care providers and to child care centers.
Provides foster care for children who have been removed from the home by a county agency as well as hard to place children (such as those with behavioral issues, disabilities, or health issues). The children receive case management (to link them to appropriate community and medical services) and individualized services from a team of professionally-trained foster families and social workers. This program is intended as a short-term solution with the goal of reunifying the primary family whenever possible. Transition support for children who are aging out of the foster care system is also provided. Recruits and trains families who are interested in providing foster care to children with special needs.

Provides Emergency Shelter Care Services for male youth between the ages of 11 and 18 who are under the custody or supervision of Franklin County Children Services. Placement can occur within one of four shelter care sites located within Franklin County. NYAP's shelter homes provide a structured setting with 24-hour supervision. Case management services are available through Treatment Coordinators who assist with referrals and linkages to meet the individual needs of referred youth. Referrals are made through the Service Reception Department and require similar referral information as indicated for foster home placement.
