Provides participants with a structured day of activities in a safe environment; breakfast, lunch and snacks are provided; for persons with Alzheimer's, a related disorder, or frail elderly. Transportation available.

Provides the following services to anyone in need, regardless of his or her ability to pay: * Medication for pain and symptom management, * Professional nursing visits, * Assistance and instruction to family caregivers, * Personal care and homemaker assistance, * Medical and social services, * Spiritual care from the hospice chaplain, * Dietary guidance from a registered dietitian, * Physical, occupational, speech and music therapies, * Trained volunteers, * Bereavement support, * Short term inpatient care, * On-call professional staff, and a supplemental paid caregiver fund.

Provides short-term temporary relief for caregivers and their families. Offers in-home care with trained staff experienced in special needs of developmental disabled individuals ages 6 months through elderly.

A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community. Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Offers older adults assistance with simple household chores and property maintenance, including weeding, mulching, raking and planting flowers.

Provides in-home assistance with personal hygiene, housekeeping tasks, food preparation, transportation, and other personal care needs for older adults.

Provides case management and coordination of needed home care services in the following areas: medical transportation, emergency response system, personal care, homemaker services, home delivered meals, and chore services, including laundry service. Services can be requested individually or combined. Can provide limited home bathroom modifications for safety. Does not include skilled nursing services. When program is full, assists those on waiting list to find needed services in the community. Chore services and limited bathroom modifications cannot be assessed without using other services.

Assists and connects older adults to in-home and community-based services to help foster and maintain their independence. Services include personal care and homemaking. No more than six hours of personal care/homemaking assistance provided per week.

Helps disabled older adults remain safe and independent at home. Prevents unnecessary nursing home placements. Consumer-Directed option gives consumers the option to serve as employer by hiring eligible individuals of their choice to provide their personal care services.

Arranges for and/or funds routine household and personal care activities for people with developmental disabilities through various programs. Services may include assistance with personal care, supervision, transportation, community activities, meal planning, grocery shopping, medical appointments and other activities of daily living.

Provides personal care attendants as well as errand-running assistance and light housekeeping services for homebound seniors.

Provides skilled nursing care that includes personal care and home health aides but only if skilled nursing is main service needed. Specialized services for cardiac, diabetic, pulmonary, and wound care are also available.

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.) Services also include personal care and homemaking for adults with HIV/AIDS.

Provides a range of home health care services including but not limited to skilled nursing services for diabetic and cardiac care, as well as assistance for older adults and those with a chronic illness or disability. Offers personal care and homemaker assistance, including bathing and grooming, light housekeeping, laundry, assistance with daily living tasks, companionship, meal planning and preparation, errand running, and transportation/escort to places such as medical appointments, shopping, and family functions. Also provides additional support, such as rehabilitative therapies and social work services.

Offers home health aide and home nursing services, including RN/LPN, case management, medication and disease process managemenet, patient/caregiver education, IV therapy, wound management, ostomy care, home health aide services, physical therapy and speech therapy. Also offers personal care and light housekeeping.
