C.T.C. opened its doors in October 1997 in Lancaster, Ohio and serves the Southeastern region. Community Transition Center believes in an individual approach to each case and uses that philosophy as a basis to assist clients with their needs. Many clients come to Community Transition Center with a myriad of issues that need attention. A comprehensive assessment of those needs is completed during the intake process. Clients complete in-house programming as well as receiving referrals to outside agencies when their needs are not within the scope of programming available at Community Transition Center. The vast majorities of clients have been involved in traditional substance abuse programming and are well aware of what working a program of recovery means. Most clients have participated in institutional programming or in-patient rehabilitation programs in the community. Although most clients come to this program directly from an institution, some have been released to the community but have relapsed to their previous drug of choice and placed at Community Transition Center in lieu of jail time. The program is very structured and provides a wide range of services, including case management, professional clinician services and trauma support groups. The 11-unit facility houses clients and their children for 6-24 months.

Tenants who sign the month-to-month lease will pay rent and are required to submit to random drug testing. A recovery coach will visit the property regularly to monitor the tenants and ensure that no drugs or alcohol is on the property.
Roommates and spouses are prohibited, although children might be permitted under certain circumstances. Tenants are expected to stay sober and participate in recovery programs. No treatment or counseling will occur at the house; rather, it's simply a place for recovering drug addicts to live in a supportive environment with others going through the same struggles.
The home is divided into four apartments in a safe neighborhood with close proximity to public transit, restaurants and laundromats on Maple Avenue.

Level 2 transitional recovery house for women struggling with substance use disorders. Women come to Serenity Grove to learn to live independently, to heal, and to gain back their lives. Built on seven wooded acres in the Ohio countryside, Serenity Grove is open to women who have completed an inpatient treatment program and/or who have at least 30 days clean.
