Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid. Completes a minimum of three (3) outreach attempts on each person screened to ensure all needs/opportunities have been met.

Also offers medically-assisted treatment and assists participants in preparing a plan for their detox after infant’s birth.

Provides a comprehensive substance and mental health program for pregnant individuals. Program includes counseling, case management, peer support, psychiatric services, and collaboration with outside providers.

Provides a mentoring program for families. Clients establish and work toward goals with a client advocate, using a video-based curriculum that is evidence based. In return, the client has the opportunity to earn vouchers which can be redeemed for diapers, clothing, and other baby items.

Assistance for Medicaid eligible pregnant women. Services include assistance with finding a doctor, scheduling appointments, transportation, and referrals to other community services.
