Provides certified copies of birth certificates to anyone born in the State of Ohio. Also acts as registrar for the filing of birth certificates for babies born in Parma but not in a hospital. Can also provide death certificates for those who have died in the City of Parma
ZMCHD can now issue birth certificates for individuals born anywhere in Ohio after 1908. This eliminates the need to visit the county of birth to obtain a certificate.
EFFECTIVE IN JANUARY OF 2011: The Richland Public Health office of Vital Statistics can now issue birth certificates for ANYONE born in Ohio. Records and issues birth and death certificates in Richland County from December 20, 1909 to present. Records for persons born or died in Richland County from 1867 to December 19, 1909 are held at the Richland County Probate Court, 50 Park Avenue East, Mansfield, OH 44902 (419) 774-5583. Our office can only issue Death certificates for deaths occurring in Richland County, excluding the City of Shelby. Records for person who died within Shelby city limits after 1909 are recorded and issued at the Shelby City Hall, 43 W. Main Street, Shelby, OH 44875 (419) 347-5131.

Death certificates can only be issued from our agency if the individual passed away in Guernsey County.

Maintains records and provides birth and death records dating back to 1909. Information on births and deaths occurring in Montgomery County prior to 1909, call the Montgomery County Archives at 225-6366.
If a birth has not been recorded, or if a birth certificate has been lost, destroyed, or improperly or inaccurately recorded, the probate court has the power, upon application, to require recording, or correction of the certificate. Information on births before 1908 is available from the probate court. For individuals born prior to 1908, a certified certificate of birth may be obtained from the probate court.
Provides same day certified birth certificate copies for all Ohio counties and certified death certificate copies for every person who died within Summit County.
