Care Consultation is a free personalized service helping families address issues that arise from Alzheimer's disease or other related dementias that can be requested by care partners/caregivers, persons living with dementia or community professionals. An individual action plan is created that can contain disease education, a safety plan, stress reduction, and any referrals to community resources needed.

Evaluates the functional ability, physical health, cognitive and mental health, and socio-environmental situation of older adults, particular those who are frail or chronically ill, to determine eligibility and refer families to appropriate agency programs.

Provides services if enrolled in their programs, a case manager helps ensure an individuals needs are met with home and community based services. These services are delivered through various case management programs and through a network of providers and may include but not be limited to help with personal care, housekeeping, preparing or receiving home delivered meals, transportation, adult day care and home medical equipment.

Provides primary care, general physical examinations, dermatology, and podiatry. Some sites offer geriatric assessments. Geriatric Evaluation is a health care visit with a team of experts that results in a treatment plan aimed at helping Veterans maintain good health, improve quality of life, reduce the need for hospital stays, and reduce the need for long term care services.

Provide in-home services as an alternative to nursing home placement for individuals of any age who meet eligibility criteria.

PASSPORT provides help at home for seniors as an alternative to nursing home care.Some of the senior services available include homemaker, transportation, personal care services, home-delivered meals, emergency response systems, adult day services, and case management. PASSPORT, providing help at home for Ohio seniors, is also referred to as Ohio's home and community based Medicaid-waiver program for older adults.

Provides needs assessment, information, socialization opportunities, and referrals to community resources for senior citizens.

C.A.R.E. offers assessments and evaluation by USS trained staff for services/programs at United Senior Services and referrals to agencies and support programs in the community.

Provides blood pressure screening and blood sugar screening.

A home care program that assists low income persons (age 60 and older) who are eligible for nursing home placement to remain in home in their community. Provides in-home assistance with personal care, homemaking, emergency response systems, meals on wheels as alternative to nursing home placement. Services for Older Adults includes Life Essentials, which has guardianship and intervention for seniors with mental illness.

Provides medical care by a physician in the home for one-time or ongoing medical conditions. Services can include primary care services and treatment for chronic conditions, one time assessments (including geriatric assessments), assistance developing a plan of care, help arranging other needed mobile health services, This is a non-emergency service.

Offers senior health support services as well as educational programs to help adults of any age learn the physical, psychological, spiritual and social aspects of healthy aging.

Provides case management to qualified individuals. Provides a detailed assessment to determine level of care needs and explanation of available services and ongoing supportive services such as monitoring and service coordination.
