Provides educational programming to help seniors manage their type 2 diabetes, arthritis, hypertension, lung disease, and other illnesses and conditions. The goal is to slow the progression ofillness as long as possible without the use of more intensive health care services. Teaches relaxation techniques, healthy eating, managing sleep and fatigue, managing medications and monitoring blood sugar, exercise, and communicating effectively with healthcare providers.

Services include depression, anxiety and mood disorder during and after pregnancy, adjustment difficulties, postpartum stress and birth trauma, treatment for substance abuse and addiction during and after pregnancy through RISE Recovery, and coping with miscarriage and loss.

Provides shared medical appointment sessions which are designed to work in conjunction with clients' primary care physicians to reverse disease for individuals who have a diagnosis of a number of chronic, debilitating, medical conditions, including lupus, psoriasis, . The program teaches how to manage and reverse chronic disease by learning stress management techniques, exercising, cooking healthfully and understanding nutrition.

Offers a 4-month program to help participants reduce blood pressure and improve blood pressure management through monthly nutrition education seminars, two personalized consultations per month, and training on how to self-measure blood pressure twice a month.

Provides a variety of services on a regular basis. Services include blood pressure and foot care. Also, provides educational tools on a variety of topics for physical and mental health.

May be able to provide clients with a limited supply of some prescriptions. Will also help clients access and complete paperwork for patient assistance programs that provide prescriptions. Established patients only, may receive referrals for on site specialty care.

Offers a Chronic Disease Self-Management Program developed for the Hispanic community. The program is highly interactive, focusing on building skills, sharing experiences, and support.

Offers a Medicaid Funded program that offers home and community based services that are person-centered and aimed at supporting individuals in the community for individuals diagnosed with a severe and persistent mental illness (SPMI) or with a diagnosed chronic condition (DCC). The Ohio Department of Medicaid (ODM) has contracted with CareSource to manage the Specialized Recovery Services program in the state of Ohio.

Helps participants’ understanding of an A1C to learning how to prepare fresh and healthy meals and to help build skills needed to control diabetes and other chronic disease in a 5-week series of classes. Recipes and fruits and vegetables are distributed throughout the program and participants who complete the program receive a 3-month membership to the Wayman D. Palmer YMCA.

Provides a workshop where those with long-lasting health conditions (diabetes, asthma, fibromyalgia, hypertension, etc.). Participants learn techniques and information to live effectively with their long-lasting health condition(s). Workshops are also effective for those managing the chronic disease of someone they care for.

Provides assistance with managing chronic diseases such as heart disease, diabetes and asthma. Diabetes management includes help with preparing healthful meals that may change the course of the disease.

Offers a curriculum to help eligible individuals take control of their diabetes and manage it. Workshop leaders provide information on medical topics, self care, fitness and ways to be physically active, cooking lessons, and nutrition information.
