Provides 24-hour emergency response to help human trafficking survivors exit trafficking. Offers case management services to assist survivors in building stability. Case Management includes safety planning, basic needs support, linkage to shelter, linkage to legal assistance, trauma therapy, behavioral health support, employment, and education, housing search assistance and rental assistance, criminal justice system advocacy. Other supportive services include trauma recovery groups, street outreach, drop in center services, and law enforcement victim advocacy.

The shelter provides emergency shelter. The residents are provided with hygienic necessities and linens. Support services to assist with obtaining income and housing.

Provides services that include child protection, crisis intervention, family assessments, specialty care/chronic illness, support groups, general; information and referral services.

Provides transitional care services to youth. Services include community referrals, assistance in locating housing, training in daily living skills and assistance with benefits. Teams help emancipate clients from the children’s system into mainstream independent living. Those with severe psychiatric disorders are helped to make a transition to the adult system of care.

Provides case management and counseling services to persons who have sustained a brain injury and have issues, or are at risk for problems, with substance use or abuse. Also offers a vocational program.

Provides mental health and substance abuse services. Offers brief, solution-focused and longer-term counseling and psychotherapy for individuals, couples, families and groups. The program also proposes Community Support Services to adults with severe and persistence mental illness. Specialty programs for adults with dual diagnosis of substance abuse and mental illness.

Program provides transitional housing for homeless families (some single women that meet the eligibility requirements) for a period up to two years. Provides a plan of action that is developed with each participant that will include case management, supportive services and a network of community services.

Provides comprehensive health care and case management to the returning Operation Enduring Freedom (Afghanistan)OEF/Operation Iraqi Freedom/OIF/OND veterans in a 13 county area in Central Ohio. Services are provided on an outpatient basis.

Serves up to 42 women each night. The residents are provided three meals a day as well as case management services. They are also provided with hygienic necessities such as showers, soap, shampoo and toothpaste. Limited clothing for the individuals is provided as well.

Program provides emergency overnight shelter for single homeless men. Evening and morning meals served.

Provides non-medical chemical dependency treatment in a residential setting for women. Program length based on need (approximately 30-90 days). Substance abuse counseling education and case management are provided.

Rapid Re-housing works directly with families to help them move from homelessness to stable housing and helping them to overcome challenges and barriers to become self-sufficient. Families in this program have been assigned to Home for Families through the centralized homeless system in Franklin County.

Provides follow up services to callers from 988 Lifeline and the 2-1-1 / Crisis Hotline who were suicidal at the time of, or shortly before the call. Follow up services are also provided to suicidal individuals who have been referred by first crisis responders in Licking and Knox Counties.

Program provides emergency overnight shelter for homeless women & children, single women, families (males and females who are married with or without children) or single males with children. Evening and morning meals are served at the shelter.

Long-term care residents (of any age with disabilities) who are willing and able to move from long-term care facilities or hospitals to qualified home- and community-based settings.

Provides supportive services to low-income veterans and their families living in or transitioning to permanent housing. Offers outreach, case management, health care referrals, legal assistance referrals and help with fee payments, daily living costs and service referrals, housing counseling services, personal financial planning services, transportation expense assistance and assistance in obtaining VA and other benefits. Additionally, the program may also provide time-limited payments to third parties (ex. landlords, utility companies, moving companies and licensed child care providers) if these payments help veteran families stay in or acquire permanent housing on a sustainable basis.

The Lifespan Transitions Center offers resources, training, technical assistance, and consultation to support the successful transition of individuals with autism and multiple disabilities throughout their school careers and into their adult lives. Resources address community living and employment.

COMPASS provides LIMITED emergency financial assistance for families/individuals facing a challenge that would affect self-sufficiency. Program provides partial assistance for emergency housing relief to prevent homelessness, allowing families to remain in their homes.

Prevents homelessness by targeting eviction prevention to those most at risk of homelessness. May provide housing problem solving, case management, and financial assistance. There is no guarantee of financial assistance availability.
