Case Management provides individualized interventions that include skill development, advocacy, linkage to medical services, housing, benefits, social activities and other community resources.

Program provides in-home alternatives to nursing home care for low-income seniors. Services include: a personal care plan to meet the individual's needs at home by arranging an appropriate mix of services to supplement care provided by family members and friends. Eligible services include adaptive and assistive equipment, assistance in the home with health activities, such as taking medications appropriately and other health-related activities, with case/care management, adult day care, chore service, home medical equipment and supplies, emergency response systems, home-delivered meals, homemaker services, independent living assistance, minor home modifications, nutrition consultation, personal care services, medical and non-medical transportation. Program also administers the Pre-Admission Review process that determines the level of care for persons who want to enter Medicaid-certified nursing homes and periodically offers vendors training on dealing with elderly/disabled individuals.

Provides case management and coordination of needed home care services in the following areas: medical transportation, emergency response system, personal care, homemaker services, home delivered meals, and chore services, including laundry service. Services can be requested individually or combined. Can provide limited home bathroom modifications for safety. Does not include skilled nursing services. When program is full, assists those on waiting list to find needed services in the community. Chore services and limited bathroom modifications cannot be assessed without using other services.

Provides a variety of services that enable individuals to remain independent and cared for at home. Included are planning for families with a member who requires an increased/increasing level of care; recommendations and referrals to address problem areas; counseling to deal with life change, loss, anxiety and depression; physical, occupational, or speech therapy; skilled home nursing; personal emergency response systems that alert a remote agency when someone has fallen and requires assistance. Service includes caregiver training.

Emergency Response System

In-home client assessments link individuals with appropriate long-term services; services are an alternative to nursing home placement which include but are not limited to adult day care, home-delivered meals, chore service, homemaker, minor home modifications, medical transportation, personal care, social work counseling, nutritional counseling, emergency response, independent living assistance, medical supplies and medical equipment.

Provides emergency response systems for persons that live alone and are disabled and/or elderly who may need to call for help in times of emergency. Has a portable lightweight transmitter that is carried and can be pressed for help.

Provides a Medicaid Waiver program that offers care management to help participants access nursing services, personal care assistance services, and/or skilled therapy services, one (1) or more waiver specific service such as: home modifications, home-delivered meals, adult day health care, respite care, supplemental transportation, adaptive/assistive devices, and emergency response systems. It is designed to meet the needs of consumers eligible for Medicaid who have been assessed to require an intermediate or skilled level of care. Without the services available through the Waiver benefit, these consumers are at risk for hospital or nursing home placement.

Provides a program to check the welfare of the citizens. Program checks-in on elderly and homebound residents via a system of computer-generated calls. The computer will call a subscriber's phone at a predetermined time, if there is no answer the software prompts the computer to place another call, if that one is not answered the computer will call a designated person. If there is still no answer the computer will generate a report to the dispatcher. A paramedic and/or a Xenia police officier would then be dispatched to the residence.

Provides a personal emergency response system for frail older adults and disabled persons. Equipment is installed and monitored by the Fire Dept.

Lifeline is a device worn or carried by an individual so they can call for help in an emergency even if they can't get to a phone. Mobile and landline units available.

Services That Assist Richland Seniors (STARS) provides in-home services and community access services and supports with funding made available through the Richland County Senior Services Levy.
