Matches experienced individuals with disabilities to our new consumers for support toward reaching specific goals. Peer support may be provided individually or in groups. Individuals with disabilities can create an Independent Living Plan (ILP) which they direct in that they choose the goals they would like to accomplish and they are guided by employees who provide advice on how to achieve the stated goals.
Provides assistance to families of children with special health care needs by helping them locate a network of quality providers and financial assistance to pay for the services needed.

Offers educational programming on topics of interest to those caring for a person with a disability or special need and publishes a guidebook of services. Offers caregiver training for both consumers and profesionals.

Consults with older adults and adults with disabilities about their needs and preferences and then screens for a wide range of appropriate government benefits and other community services that they may be eligible for. Will assist the consumer with gathering any necessary documents and will guide them through applications process (including assistance with filling out forms) as needed. Agency provides follow-up with the client. Goal is to minimize the number of calls and contacts the consumer has to make, and encourage the use of the least costly and least restrictive services.
