The SSA meets with each person and develops outcome-based Person-Centered and Self-Directed Plans. This means help with identifying needs, finding providers to support those needs (such as in home support, employment, etc) scheduling doctor's appointments, finding housing, finding transportation for appointments and other supports a person needs to live their life.
